This Release provides a brief benefit comparison of UNICARE Participating Provider (PPO) plans that are available to you and your employees. (PRWEB) September 12, 2004 -- These Benefits at a glance are a comparison intended to help you compare Unicare Plan benefits for independently contracted in-network providers, and reflects your share of costs for covered expenses after you have met any applicable deductibles.
1.All plans feature a $5,000,000 per member lifetime maximum in benefits. 2.When you use UNICARES independently contracted in-network (participating) providers, your costs are based on a specially negotiated fee for UNICARE which ultimately should result in less out-of-pocket cost for you. 3.When you use out-of-network (nonparticipating) providers, your costs are based on covered expenses, and often result in higher costs to you (but less than having no insurance coverage at all). 4.Most plans offer $30 copay for Dr. office visits with the deductible waived for the first 4, or 10 visits. Different percentages apply after deductibles are met depending on the plan of choice. With the Performance Plus having no deductible and unlimited visits! 5.MedCall (trademark reg.) is a Toll-Free health information hotline by phone to health plan members. 6.UNICARE Healthy Extensions Discounts are also available to all health plan members. 7.Prescription Drug Benefits with a $10 copay for generic drugs. Then a $25 copay for name brand drugs (most plans have a separate deductible for name brand RXs) depending on the plan of choice.
