This research study confirms the clinical benefits of continuous waveform CO2 monitoring using Oridion Microstream® capnography to reduce the incidence of hypoxemia, severe hypoxemia, and apnea in procedural sedation. These benefits persisted even after oxygen supplementation.
The aim of the study was to examine whether intervention, based on information from capnography monitoring of patients under sedation for gastrointestinal endoscopy procedures, could reduce the frequency of hypoxemia and apnea. The study analyzed a total of 247 patients undergoing ERCP and EUS procedures in a randomized, double blinded design using the Oridion Smart CapnoBloc™ ventilation management system (CO2 sampling incorporated into a bite block) and the Capnostream™20 dual parameter monitor.

Results demonstrated that capnographic monitoring of respiratory activity significantly reduced the frequency of hypoxemia, severe hypoxemia, and apnea. The benefit of capnography persisted even after oxygen supplementation.

The authors concluded that capnography monitoring functions as an early warning system for hypoxemia and may further improve patient safety during gastrointestinal endoscopy.
Dr. John Vargo MD, MPH and Mohammed Qadeer MD, MPH from the Digestive Disease Institute at the Cleveland Clinic Foundation, Cleveland, Ohio, were the lead clinical investigators on the research project. "This is the first data in the endoscopic literature to show a benefit for capnographic monitoring in patients undergoing prolonged endoscopic procedures such as ERCP and EUS" Vargo said.    "The study also confirmed some of our previous observations that significant alterations in ventilation can occur without oxygen desaturation. In fact, capnographic abnormalities frequently precede the development of hypoxemia."
