Plexis finalizes agreement with On Lok Senior Health Services

Plexis Healthcare Systems, Inc., one of the nations leading healthcare information systems providers, has signed an agreement with On Lok Senior Health Services. On Lok is the prototype site for the emerging PACE (Program of All-Inclusive Care for the Elderly) model for long-term health care delivery and financing, which has been successfully replicated at over 25 sites across the United States.

About PACE
The PACE program is a unique capitated managed care benefit for the frail elderly provided by a not-for-profit or public entity. The PACE program features a comprehensive medical and social service delivery system using a multidisciplinary team approach in an adult day health center that is supplemented by in-home and other support services in accordance with participants' needs. PACE programs enroll only older people frail enough to meet their states' eligibility standards for institutional (nursing home) care. Today there are more than 70 organizations in 30 states working in various stages of the PACE model. In the Balanced Budget Act of 1997, PACE became a permanent provider type under Medicare, and states were granted the option of paying a capitation rate for PACE services under Medicaid.
