Elderplan Expands Provider Network with Signing of
Heritage New York Medical Group
GARDEN CITY and BROOKLYN, N.Y. -- As part of Elderplans ongoing expansion efforts aimed at increasing member choice and provider satisfaction, the organization has signed an agreement with Heritage New York Medical Group, an Independent Practice Association (IPA) which contracts with a broad network of health care providers throughout the New York metropolitan area.
The relationship adds more than 600 doctors at 700 locations to the Elderplan provider network and consists of both primary care physicians (PCPs) and specialists.

Since Elderplan was developed in 1985, we have worked diligently to add more and more providers to our network, giving our members increased choices for their health care."
Elderplan is a not-for-profit Social HMO serving seniors in Brooklyn, Queens, Manhattan and Staten Island.

Both members and existing providers will benefit from the agreement between Elderplan and Heritage. Elderplan members will have more PCPs to choose from, and current Elderplan network providers will have a wider selection of specialists to which they can refer their patients who are covered by Elderplan.
