FREE CARDIOVASCULAR HEALTH SCREENING EVENTS AND RISK REDUCTION PROGRAMS, New York City Sponsored by Councilman Allen Jennings & Community Agencies (Queens, NY)

HealthReach NY, a non profit, community based healthcare organization, based in New York City, joins Councilman Allan Jennings in conducting FREE cardiovascular health screening events this month. The screenings will consist of Blood Pressure Checks, Weight and Height measurements, Cholesterol and Blood Sugar Screening.

In response to the increase of Heart Disease in the African American Community, Councilman Allan Jennings Jr. along with HealthReach NY and Community Health Network will be conducting FREE cardiovascular screening events this month. The screenings will consist of Blood Pressure Checks, Weight and Height measurements, Cholesterol and Blood Sugar Screening.

When: January 28, 2003, Tuesday, 5:00pm -- 9:00pm

Patients will complete a brief risk assessment questionnaire and receive education materials on healthy lifestyles. Those at risk for heart disease will be advised on medical care and risk reduction programs (nutrition, diabetes management, exercise program, etc). Those patients found to be at moderate to high risk will be offered a case managed program offered by HealthReach NY.

A New Year is a new beginning, that offers an opportunity to begin with better health habits," commented Councilman Jennings. The free Health Screening provides everyone with the opportunity to begin a healthy new year."

HealthReach NYs mission is to make a full spectrum of health care resources accessible to the uninsured and under-served adult populations in Queens. HealthReach NY seeks to improve the access of health care of this population through a network of healthcare providers willing to provide free medical care in private offices and institutions, extensive collaboration with community organizations, and through the provision of community health education.

Community Health Network is a nonprofit organization that provides comprehensive community-based primary care, mental health care and social services for low -income people in underserved communities throughout the City of New York. CHN is affiliated with New York Presbyterian Healthcare system and maintains a special relationship with NYC Health and Hospitals Corporation.
