The Wizdom TransPlanner helps students with disabilities, their parents, and case managers effectively plan the transition from school to life after graduation. Via the internet/intranet through all phases of transition planning, TransPlanner automatically develops active Individual Transition Plans (ITP) for students with disabilities. The Illinois Continuous Improvement Plan for Special Education, which TOTAL supports, places great emphasis on secondary transition outcomes for students with disabilities, including increased rates of graduation, employment and enrollment in post-secondary education or training.
According to Camille Mayhall, M. S., CRC, Transition Specialist/Supervisor, Department of Human Services, Office of Rehabilitation, Niles Township High Schools, ÂThe Office of Rehabilitation Services has just laid out our mission. We will return to serving students with the greatest needs, and the TransPlanner!, along with the TOTAL best practices, will help us meet the needs of more and more students.Â
ÂThe response from those who are taking advantage of the hands-on demonstrations is outstanding, said Philip J. Vitkus, Wizdom Director of Instructional Services. ÂGetting information about available, services, and helping students with disabilities and their parents create the best plans for the future and get the best results is what we are all about. And those who understand transition planning really Âget it  they can see how using the TransPlanner! will help get the best results.Â
