Statement of Need:
The redefinition and expansion of bipolar disorder to include a broad spectrum of manic states and dysphoric symptoms has dramatically increased prevalence rates from 1% (previous estimate) to around 5%. Over 40% of all bipolar episodes are mixed states. Furthermore, by some estimates, 30-55% of all major depression lies along the bipolar spectrum. Bipolar II is characterized by hypomania and episodes of depression. Bipolar III refers to antidepressant-associated hypomania. Mixed states and dysphoric symptoms may be clinically challenging to characterize, resulting in significant diagnosis and treatment delays. Atypical depressive symptoms, anxiety states, and comorbid illness (including substance/alcohol abuse) may further confound diagnosis and treatment. In this evidence-based psychCME TV activity, experts will discuss how to recognize the entire clinical spectrum of bipolarity in order to maximize diagnostic accuracy, treatment efficiency, and thus patient outcomes.

• Identify and differentiate between the different subtypes of bipolar disorder.
• Discuss potential atypical presentations of bipolar disorder, including mixed versus manic states as well as symptoms of depression and anxiety.
• Recognize factors that may confound diagnosis including comorbid conditions.
• Design treatment regimens with both psychotherapeutic and pharmacological aspects that efficiently manage a spectrum of symptoms.
