CivicPlus will work with the winning organization to design an extremely professional, turnkey E-Government Website. The winner will also receive training on updating and managing the site. Support and hosting for a 2-year service period will be included.

2. Visit www.civicplus.com/makeover and download the entry form in Word format or in PDF format. You may send the completed form to CivicPlus via e-mail, postal mail, or fax. This information is located at the bottom of the form.
Opening date for entries is October 1st, 2005. Entries submitted before this date will be deleted.
Closing date for entries January 31st, 2006. Entries submitted after this date will be deleted.
