EPOCH Home Care Expands Service Area; New Office Services Norton, Sharon and Surrounding Communities

EPOCH Home Care, a division of EPOCH Senior Living, announced that it has opened a home care office in Norton. The new office is located at 190 Mansfield Ave., Norton (508-285-3746), and serves patients in areas surrounding Norton and Sharon, including towns within Bristol and Norfolk Counties.

Norton, MA (PRWEB) January 12, 2008 -- EPOCH Home Care, a division of EPOCH Senior Living, announced that it has opened a home care office in Norton. The new office is located at 190 Mansfield Ave., Norton (508-285-3746), and serves patients in areas surrounding Norton and Sharon, including towns within Bristol and Norfolk Counties.

EPOCH Home Care received certification from the Centers for Medicare and Medicaid to provide an extensive line of services to seniors living at home, in assisted living facilities, and patients who are discharged from hospitals and short-term rehabilitation facilities and are transitioning home. EPOCH Home Care opened its first office in Waltham in January, 2007.

EPOCH Home Care has added many patients who are taking advantage of EPOCH's unique connection between its skilled nursing facilities and the Home Care staff. Under this arrangement, patients who come to EPOCH's facilities for short-term rehabilitation therapy and other services and select EPOCH Home Care as their home care provider are transferred home where they can continue rehab services with the same group of therapists.

EPOCH Home Care includes registered nurses, physical, occupational, and speech therapists, home health aides and medical social workers. Working together with physicians, these health care professionals create and implement a coordinated health care plan for patients receiving care at home. Cheryl Blake, R.N., is the Clinical Director for EPOCH Home Care.

EPOCH Home Care provides Medicare-certified skilled nursing services, such as wound care, IV therapy, and diabetes management; and, physical, occupation and speech therapies, medical social services and home health aide services.
