Qtrac stands out as a true innovator in long-term care clinical management software

Qtrac Software, developer of Clinical Management Software for the long term care industry and Patient Placement Systems, a national provider of patient referral software to the healthcare industry, today announced a partnership to help long term care facilities efficiently manage patient referrals and intake and seamlessly transition to an Electronic Health Record designed to deliver best in class quality of care.

With Patient Placement System's experience with hospitals and other care providers and Qtrac's deep vertical industry experience in Long Term Care, this partnership provides a unique opportunity to develop innovative solutions that facilitate an efficient and accurate integration of patient referral, admission, document management and care delivery. Through this partnership, Patient Placement Systems will connect disparate referral, admissions and discharge databases with Qtrac's clinical care applications to streamline communication, transfer of data and enable new residents to be quickly and efficiently transitioned into a managed care delivery model with supporting documentation.

Daphne Clark, president of Qtrac Software, said, "Patient Placement Systems software is recognized as an industry leader for patient referral automation solutions, we are pleased to be working with them to create one of the first solutions that will connect long term care admission processes and care delivery management systems. And are especially excited to be able to offer a solution to our facilities that will allow them to reduce redundant administrative intake paperwork and allow good quality care to be delivered to new residents as soon as possible."

"Qtrac stands out as a true innovator in long-term care clinical management software," said Eric Christ, president of Patient Placement Systems. "Both companies are dedicated to improving long-term care business and clinical processes, and this partnership reflects our desire to merge leading solutions make admissions and clinical management more effective and productive."

How the integrated solution works: A continuing care provider receives a patient referral by fax, phone or electronically in the PatientPlacement.com Referral Management System. The admissions team receives an alert and assigns and tracks review tasks for the referral. Once the patient is accepted, the admissions or clinical staff selects the Qtrac option from a drop down box of data formats. The staff can import the data directly into the Qtrac system to use as the basis for an electronic medical record and patient clinical management.
