The Incident and Hazard Public Web Forms allow various stakeholders (employees, contractors, visitors, guests etc.) from an organization to report an incident or hazard via a simple web form. Stakeholders simply go to the web to report the incident or hazard. No login, username or password is required. The incident and/or hazard data is automatically transferred to IndustrySafe Safety Management Software, and IndustrySafe users are automatically notified by email of the incident or hazard.

While stakeholders are provided with a simple web form to complete, the Public Web Forms are connected to the IndustrySafe Safety Software and IndustrySafe System Administrators can configure these forms to match the needs of their organization. IndustrySafe System Administrators can add, edit, and disable fields on the Public Web Forms, modify drop downs, and include their own logo to allow for quick and easy reporting of incidents and hazards.

The Public Web Form displays fields from the IndustrySafe Safety Software Incident Investigation Form and Hazards Form with the configurations already made to these forms included. System Administrators can determine if the Public Web Forms should display all or only some of the fields from the IndustrySafe Incident Investigation form and Hazards Recording Form.
