Florida Health Care Association Selects HealthScreen Disease Management as a Preferred Vendor For Member Organizations

HealthScreen Disease Management, an independent health risk management company specializing in clinical/claims based cost risk analysis along with personalized nurse care, announced today that it has been selected by the Service Corporation Board for the Florida Health Care Association (FHCA) as a preferred vendor.

The Association, based in Tallahassee, is Florida's first and largest advocacy federation for long-term care providers and the elderly they serve. FHCA represents over 1,000 members and over 500 facilities, with more than 20 active committees. The FHCA Service Corporation board evaluates and recommends companies that can offer their members reduced rates and/or enhancements on the purchase of essential products and services.

HealthScreen Disease Management provides population health management programs combining disease management, wellness/prevention, education and health culture assistance for employer groups, as part of their employee benefit program offerings. HealthScreen engages individuals to participate in both "at-risk" and chronic care programs with telephonic registered nurse care managers who actively coach, motivate and promote self-empowerment toward positive behavioral change for a healthier lifestyle. With a healthier employee population, employers see reductions in absenteeism, increased productivity, less turn-over and greater morale.

FHCA member facilities and organizations will have an opportunity to receive on-going Healthcare Cost/Risk Analysis reports, activity measures and outcomes reporting to monitor the impact of expected health improvements as it translates toward reduced claim costs and positive return on investment. HealthScreen has agreed to provide a Net Financial Guaranteed Return on Investment with fees at risk for FHCA members who participate.
