Patient Placement Systems announced today that Visiting Nurse-Hospice Atlanta, the largest nonprofit home healthcare and hospice provider in Georgia, has implemented the Enterprise Edition of its Web-based Referral Management System to automate the home health and hospice intake process and to manage the large daily fax volumes, in one integrated solution. The system is in use at Visiting Nurse-Hospice Atlanta home health locations, at Hospice Atlanta, and at Visiting Nurse Care Management Services.
The Referral Management System automates and accelerates patient referral intake for post-acute care providers, replacing slow fax-and-paper processes with a fast, simple and efficient online software service. During the first phase of implementation, Visiting Nurse-Hospice Atlanta has converted nearly 5,000 monthly faxes and 20,000 fax pages into electronic documents for easy online review, tracking and archiving. Visiting Nurse-Hospice Atlanta is currently rolling out additional referral tracking features, including insurance verification, enabling the intake teams to instantly confirm insurance eligibility on the more than 100 patients referred daily from acute care hospitals, physician offices, and other providers.

Patient Placement Systems also partnered with Visiting Nurse-Hospice Atlanta to add new capabilities to the Referral Management System:
- Electronic Signature: The Referral Management System enables users to electronically sign PDF documents, identifying who signed the document and when. It also verifies that the document has not been changed since signing. This added functionality simplifies the approval of faxed documents during the intake process.
- Interface with McKesson Horizon Homecare: Patient Placement Systems developed an interface that automatically and continuously pushes patient referral information from the Referral Management System into the McKesson Horizon Homecare system, used by Visiting Nurse-Hospice Atlanta for home health and hospice operations. This interface, built according to HL7 standards for healthcare data exchange, ensures data accuracy and consistency across the two critical systems and eliminates rekeying of patient information.
- Enhanced Document Management: Based on feedback from Visiting Nurse-Hospice Atlanta, Patient Placement Systems added new features to the fax management module, allowing users to easily transfer documents from one team to another. This, along with expanded document folder capabilities, has raised RMS to an enterprise fax server level.
