CentraState Healthcare System Introduces MedsTracker Software by Design Clinicals for Patient Medication Safety; Sees Drastically Reduced Reconciliation Times

CentraState Healthcare System in Central New Jersey implemented MedsTracker software by Seattle-based Design Clinicals and drastically reduced medication reconciliation time to under three minutes. MedsTracker improves speed and accuracy of medication reconciliation for clinicians while helping patients better understand and comply with their medication regimen.

CentraState Healthcare System, a not-for-profit health organization in Central New Jersey, contracted with Seattle-based Design Clinicals, Inc., to implement MedsTracker for patient medication management in its 282-bed acute-care medical center. Since go-live December 7, 2009, CentraState has shaved the formerly laborious process of medication reconciliation to under three minutes, with a current median discharge reconciliation time of just one minute 38 seconds.

“We were confident implementing MedsTracker would save time as well as increase medication safety,” said Indranil (Neal) Ganguly, Vice President & Chief Information Officer for CentraState. “The results are even better than we had anticipated.”

MedsTracker captures a detailed medication history for each patient and provides alerts for potential allergy or drug interactions with any changes to the medication regimen. Patients receive printed lists of their medications with detailed information to help improve understanding and compliance. MedsTracker has been shown to reduce medication errors and can save critical time for first responders in emergency situations.

Implementation of MedsTracker puts CentraState Healthcare System well ahead of the mandated medication reconciliation requirement stated in The Joint Commission’s National Patient Safety Goals. Hospitals and healthcare systems around the nation continue to struggle with the complex issue of medication reconciliation as they move from paper to electronic systems.

It can automatically fax a prescription to a patient’s pharmacy of choice, and eliminates errors due to illegible handwriting. It helps ensure the highest standards of patient medication safety while being user-friendly for physicians and registered nurses.
