With input from sixty-four (64) healthcare organizations, the Agency for Healthcare Research and Quality (AHRQ) has developed common formats for inpatient related safety incidents. Health delivery organizations reporting Patient Safety events can now quantify these events using these newly standardized formats.

The challenge for hospitals is to develop a way to track and trend patient safety incidents using this standardized reporting method - including a comprehensive flow of questions and complex if/then logic - into a solution flexible enough to support a wide range of real-world safety issues.
