Planning for the post-hospitalization care needs of a senior prior to discharge will help ensure a smooth transition to life at home and support family members while reducing the risk of complications, explains Steve Barlam, Chief Professional Officer of LivHOME.

But planning in advance for his or her post-discharge needs will ease the burden considerably. The first step is a a discharge plan mapping out practical issues such as medication schedules, diet, exercise, therapies and any equipment these require. The second step is a post-discharge plan coordinating the implementation of this care. The third step -- organizing a team of family members, community members and professionals to help implement -- should ideally be taken during hospitalization, if not before,” says Barlam.
Barlam adds that planning ahead frees up family members to provide the emotional support critical to healing and supports a senior’s recovery by ensuring that no care requirements slip through the cracks.
