URAC Expands Patient Centered Health Care Home Program with Release of Auditor Certification

Certification Prepares Auditors to Review Practices Transforming Into Health Care Homes; Assures Providers of Professional Evaluation.

URAC, a leading health care accreditation and education organization, announces the release of its Patient Centered Health Care Home (PCHCH) Auditor Certification. This extension of URAC’s overall PCHCH program will meet the needs of those entities whose responsibility will lie in independent auditing of health care home practices to determine their degree of successful achievement of the PCHCH standards.

URAC is developing a step-wise program to guide practices as they transform into truly patient centered health care homes that focuses on the integration of family, caregivers, patients, and care coordination in the delivery of quality health care. To educate practices in becoming health care homes, URAC released the Patient Centered Health Care Home Program Toolkit and related Information resources in December 2010.

Now, with this new PCHCH Auditor Certification, URAC supports third party reviewers in evaluating practices to determine if they are delivering on the core elements of a patient centered health care home. Additionally, URAC certification will assure practices that such independent audits are being performed by qualified health professionals.

URAC’s PCHCH Auditor Certification is targeted at health care management organizations such as medical home pilots, health plans and practice consulting organizations among others. Verified audit information can then be used as a component of pay for performance programs, evaluation of practices as health care homes by a health plan or in pilot demonstration projects.

The URAC PCHCH Auditor Certification program encompasses URAC's Core Module Standards which evaluate an organization's operations, training, processes and systems. Upon completion, an organization receives Core Accreditation and PCHCH Auditor Certification.

The URAC Patient Centered Health Care Home Auditor Certification Includes:

  • Core Accreditation Standards v3.0.
  • HIPAA Business Associate Requirements.
  • Auditor Eligibility Requirements.
  • Auditor Inter-Rater Validation of URAC-issued Practice Audit Tool.
  • URAC's PCHCH Practice Assessment Toolkit - required for conducting the practice audits.
