CareSource received a Pinnacle Award at the 2011 Ohio Association of Health Plan’s (OAHP) Annual Convention in May. CareSource was one of three winners with the top honor in the Health Care Programs category for their Neonatal Intensive Care Unit (NICU) Care Transitions Program. The Pinnacle Awards recognize the efforts of health plans to improve the quality of health care for Ohioans.
With 4 out of 10 babies in Ohio born into Medicaid, a focus on healthy babies is critical. Certain populations are particularly at risk to have premature babies that require long stays in Neonatal Intensive Care Units. Uncoordinated and fragmented care leads to poor outcomes including longer, more costly hospital stays and readmissions. With that in mind, CareSource created its NICU Care Transitions Program to safely send high risk infants home, to lower utilization and reduce hospital stays.

The program pairs experienced NICU Case Managers with parents and providers to create a comprehensive transition plan.”
OAHP invited all member plans to participate in the competition, which provided an opportunity for health plans to demonstrate what they are doing to provide exemplary care and service for their members and community. A panel of experts representing hospitals, doctors, businesses, consumer advocates, and regulators judged the submissions and selected the winners.
