Launches Software-As-a-Service (SaaS), Telehealth Solution Designed to Improve Patient/Physician Communication and Care

Chronic Care Management, Inc. offers a total telehealth and telemonitoring solution to improve patient care plan compliance and create new opportunities in patient education and outreach with TeleHealth.

The launch of the fully integrated telehealth solution gives health care providers the power to communicate with patients and monitor their vital signs (blood pressure, heart rate, glucose and weight) using the VirtualHealthCheck telehealth center in San Antonio, Texas.

We offer patients three ways to store and share their vital sign information. They can enter their data into our mobile apps or, or use our in-home, wireless monitoring systems. Our system even alerts health care providers when vital signs go out of range. Alerts can trigger a multi-channel telehealth response to communicate with patients via inbound/outbound phone, e-mail and/or live chat.

Providers can also manage communication between our telehealth agents and patients, create surveys and establish call rules for specific patients or groups of patients based on care plan needs.
