Case Management Professionals Play Key Role in Patient Care

Case managers provide a valuable role assisting patients and their care teams as well as the leadership of hospitals and health systems.

Case management plays an important role in the success of patient care for today’s healthcare community, according to the American Case Management Association (ACMA), which observes National Case Management Week on Oct. 14-20, 2012. Organizations, including Allsup, a nationwide provider of Social Security disability representation and Medicare plan selection services, recognize the dedication, compassion and achievements of case managers within hospital and health systems settings.

The association’s theme for 2012 National Case Management Week is “Patient-Oriented, Outcome-Driven.” The organization focuses on the needs and concerns of case managers in hospitals and health system case management. The week provides an opportunity to recognize the role of case managers in the delivery of high-quality and cost-effective healthcare services to patients and their families.

Case managers provide clinical assessment, patient education, counseling and care coordination, case monitoring and clinical pathway management, discharge planning, resource management and patient advocacy. “Case managers provide a valuable role assisting patients and their care teams as well as the leadership of hospitals and health systems,” said Karen Hercules-Doerr, director of Community Based Representatives at Allsup.

ACMA will provide resources and tools to case managers during its 2012 Leadership Conference on Nov. 8-9 in Austin, Texas. The event will feature faculty from the Institute for Healthcare Improvement, an independent, not-for-profit organization based in Cambridge, Mass. The conference theme is Reducing Readmissions, Refining Patient Flow. The conference is designed specifically for hospital case management leaders.
