The URAC 2012 Best Practices in Health Care Consumer Empowerment and Protection Awards recognize premier healthcare management programs that demonstrate quality improvements in healthcare delivery. Nurtur® is proud to join with its parent company, Centene Corporation, in receiving the URAC 2012 Best Practice Platinum Award for its Asthma Solutions Program for a Managed Medicaid Population.
Nurtur began working in partnership with Centene in 2007 to develop an integrated disease management program to provide risk-stratified asthma case management focused on prevention, education and wellness. The Nurtur/Centene Asthma Solutions program includes telephonic outreach, home visits, culturally-sensitive and age appropriate educational materials, environmental interventions and community engagement. It is delivered based on the Nurtur PeopleCare™ exclusive patient-centered approach to behavior change to help individuals manage both health and life issues. Aggressive care coordination ensures patients have basic services such as transportation to the doctor and pharmacy, electricity to power the nebulizer, and a clean, safe home environment.
Program outcomes from a two-year study October 2009 – September 2011 demonstrated estimated cost savings at over $13 million annually. Results included 13.4 percent reduction in asthma-related ER visits and 12.7 percent reduction in all respiratory-related ER visits as well as 56.7 percent increase in pulmonary function testing. In addition, the study demonstrated improved preventative care by program participants with 10.3 percent increase in visits to primary physicians, 18.4 percent increase in flu shot vaccinations and 16.8 percent increase in controller medication prescriptions as compared to non-participants.
