There are less than two weeks left to nominate volunteers to win the CTK Foundation's Heroes with a Heart community service grant award program, celebrating community service volunteers who have exceeded all expectations in giving of heart, mind and hands.
Heroes with a Heart Community and Human Services Grant Qualifications
Both the nominating and nominated parties must be affiliated with a 501(c) 3 or registered charitable organization. The $5,000 Health and Human Services Grant is open to any volunteers who freely give their time to protect health and provide essential human services.
Total cash grants (USD) to selected volunteers will equal $17,500 (USD) and additional video awards are valued at $30,000.
Health and Human Community Service Grant
A $5,000 CTK Foundation Health and Human Community Service Grant will thank the selected volunteer, and a professionally produced video will be made about the affiliated non-profit for use in public awareness or community service education.

$5,000 CTK Foundation Environment Grant or Animal Welfare Grant
$5,000 CTK Foundation Arts and Literacy Grant
$1,500 Susan Lee Winter Grant Award for Innovation in HIV/AIDS Education or Prevention
$1,000 CTK Foundation Blogger’s Choice Award.
Nominate Your Community Service Hero
Nominations for CTK's community service hero grant awards are open now and applications close Wednesday, February 29th, 2012, at midnight (all time zones). Self-nominations will not be accepted and grant applications must be completed and submitted according to directions and schedule in order to be considered. CTK Foundation judges will select semi-finalists after reviewing all applications. From March 15th to April 15th, 2012, at midnight, semi-finalist stories will be posted for voting on Facebook. Winners will be announced in early May.
