Concierge Care, a full-service home health and discharge-planning program, utilizes Healthcare Solutions’ in-house nurse specialists to assist customer case managers in identifying and coordinating all necessary home care services for each injured worker. Through the program, Healthcare Solutions’ nurses coordinate with discharge planners and case managers to facilitate each injured worker’s smooth transition from the hospital to a home care setting. This coordination includes identifying qualified nursing agencies to deliver home care and recommending appropriate durable medical equipment and home modifications to help injured workers avoid complications or delay in care.
