CDPHP® is back in the national spotlight. Director of clinical transformation, Lisa Sasko, is set to address a health care conference in Washington, D.C. next week. Sasko has been called upon to discuss the CDPHP Enhanced Primary Care (EPC) initiative, a patient-centered medical home program that is literally transforming the field of primary care in New York’s Capital Region.
In 2009, CDPHP launched EPC with the goals of improving health outcomes for its members, while achieving cost-savings and patient/physician satisfaction. The program was also designed to address the shortage of primary care physicians by making the practice more financially viable. The program now includes 73 local physician practices, encompassing close to 100,000 CDPHP members and nearly 384 network physicians.
An independent analysis of the program produced the following impressive results:
o 21% reduction in inpatient admissions.
o 9% reduction in emergency room visits.
o $8 per member per month savings.
Sasko will be joined by Dr. Holly Cleney of the Latham Medical Group, whose practice is participating in the EPC program. Sasko and Dr. Cleney are scheduled to speak at the Summit on Payment Reform and Delivery Transformation in Washington, D.C. on March 5.
