“This is a big win for the Capital Region, the Hudson Valley, and CDPHP,” said Bruce Nash, MD, chief medical officer, CDPHP.
Albany, NY (PRWEB) April 14, 2012
CDPHP® is excited to announce that New York’s Capital District and the Hudson Valley have been selected by the federal government to transform the delivery of primary care throughout the country. The region is one-of-seven markets in the nation that has been selected by the federal government to participate in the Comprehensive Primary Care initiative (CPC), which will test payment models to improve the quality and efficiency of health care.
CPC is a groundbreaking initiative launched by the Center for Medicare & Medicaid Innovation (CMMI). The initiative invites payers to invest in primary care doctors who are committed to care coordination. Primary care practices that are selected by the government will be given financial incentives and other resources to better coordinate primary care for their Medicare patients.
“This is a big win for the Capital Region, the Hudson Valley, and CDPHP,” said Bruce Nash, MD, chief medical officer, CDPHP. “We began the work of practice transformation and payment reform five years ago when we launched our Enhanced Primary Care initiative. The program now includes 75 local physician practices, encompassing nearly 100,000 CDPHP members and nearly 384 network physicians. I am confident the work we began five years ago played a significant role in the federal government’s decision, and I’m eager to accelerate the transformation of care in New York’s Capital District and the Hudson Valley,” added Nash.
Nash says an independent analysis of the CDPHP’s Enhanced Primary Care initiative produced the following impressive results:
- 15% reduction in inpatient admissions.
- 9% reduction in emergency room visits.
- $8 per member per month savings.
Below is a list of the seven regions selected for the CPC initiative:
- Arkansas: Statewide
- Colorado: Statewide
- New Jersey: Statewide
- New York: Capital District-Hudson Valley Region
- Ohio: Cincinnati-Dayton Region
- Oklahoma: Greater Tulsa Region
- Oregon: Statewide
