Dorland Health Announces its 4th Care Coordination Summit: New Delivery Models Transforming Healthcare

On September 14, 2012, Dorland Health will host an interactive learning event for all healthcare professionals responsible for the financials and quality improvement related to care coordination

Seeking to provide solutions to the complex issues facing healthcare teams across the delivery system, Dorland Health announces the 2012 Care Coordination Summit, an interactive learning event designed for all members of the care coordination team as they strive to optimize their business practices and quality-of-care initiatives.

Bringing together a diverse group of speakers from all slices of healthcare, this all-day Summit is designed to shed light on issues related to the finances and quality of care in the expansive areas of care coordination. Seeking to improve quality and reduce costs, the Summit focuses on some of the top-trending areas that impact modern healthcare delivery, including value-based purchasing, medical homes, accountable care organizations, transitions of care, dual eligible patients, patient engagement, health information exchanges and more.

“The Care Coordination Summit delivers insights, case studies and actionable strategies from frontline leaders working across health plans, hospitals, nonprofits and other points of care that leaders in care coordination can use to improve their practice, positively influence the health of their organization, combat rising cost shares and penalties, and advance the delivery of care,” said Anne Llewellyn, RN-BC, MS, BHSA, CCM, CRRN, editor in chief of Dorland Health and chair of the event.

The day’s discussions will be punctuated by two keynote addresses – the morning keynote by Dr. Jesse Gruman, president of the Center for Advancing Health, and the afternoon keynote by Patrice Tanaka, co-founder and chief creative officer of CRT/Tanaka.

The 2012 Care Coordination Summit covers practical topics for professionals working in the field of managed care, acute care, the military, long-term care, behavioral health, rehabilitation, home care, or other settings through which consumers access healthcare. The only event designed for all members of the care coordination team, the Care Coordination Summit provides participants with the insight, strategies and tactics to improve the performance of their organization so as to improve the quality of care while reducing costs.

The Dorland Health Care Coordination Summit will be held Friday, September 14, from 8:00 a.m. to 5:00 p.m. at the National Press Club in Washington, D.C.
