Nurtur®, the behavior change company serving multiple market segments, was honored to receive the “Outreach Award” at the second annual Awards Forum hosted by Medicaid Health Plans of American’s (MHPA) Center for Best Practices. This prestigious award forum honors Medicaid health plans for innovative and effective best practices that have improved the health of Medicaid enrollees by ensuring high-quality care.

Nurtur was one of only six healthcare organizations recognized at the event held in Washington, D.C. Many prominent organizations were among the14 finalists for the Outreach Award, a category that recognizes the best examples of plans that take proactive steps to find enrollees and help them overcome barriers that often impede the delivery of healthcare.

Nurtur’s Diabetes Program provides health coaching and support to participating members through telephonic outreach and educational materials designed to address life barriers, improve self-management skills and promote adherence to prescribed treatment. Members are identified for enrollment based on medical and pharmacy claims data, by self-referral, or by referral of a health plan physician or case manager. Once enrolled in the program, members receive creative tools and educational materials suited to their age and literacy level and delivered by Health Coaches, who are Certified Diabetes Educators. Some of the tools used include pre-programmed cell phones loaded with a Diabetes Management book as well as podcasts on wellness and self-managing chronic disease.
