Dorland Health’s fourth Care Coordination Summit: New Delivery Models Transforming Healthcare, an interactive, full day learning event will be held September 14th, 2012 at the National Press Club in Washington, D.C.

The Dorland Health Care Coordination Summit, the only interactive learning event designed for all members of the care coordination team, delivers insights, case studies and actionable strategies from frontline leaders working across health plans, hospitals, nonprofits and other points of care to help organizations meet the challenges of the 21st century healthcare system.

Sessions throughout the day feature a number of expert faculty members with decades of collective experience from top-name organizations that are working at the point of care and creating in their own settings an environment that is patient- and family-centered, safe, efficient and cost-effective. The faculty will help the audience explore cutting-edge ways organizations are redesigning models of care, grasping the opportunities of health reform, creating cultures of continuous improvement, and engaging patients through collaborative communities across the continuum of care.

The opening keynote will be Dr. Jessie Gruman, President of the Center for Advancing Health. Dr. Gruman will cover patient engagement from the most important perspective –that of the patient. She will discuss the challenges patients face in engaging in their care as well as approaches to overcoming some of the biggest obstacles that stand in the way of comprehensive engagement.

For those interested in enhancing patient advocacy skills, the Professional Patient Advocate Institute will present an all-day workshop on Thursday, September 13th that will earn participants, along with some additional coursework, a certificate in Patient Advocacy. The Certificate Program is an advanced course that is open to all professionals with a baseline level of healthcare experience. Additionally, the Certificate Program serves as a point of entry for professionals who want to transition into this new role, providing a course of study that builds on an existing body of knowledge.
