The research identified six imperatives providers must consider when planning and developing primary care strategy and infrastructure for the communities they serve. The imperatives are:

1. Managing risk payments
2. Achieving performance improvement and financial viability for employed primary care physicians
3. Determining primary care network size, mix, and geographic distribution
4. Implementing primary care recruitment and retention strategies in highly competitive markets
5. Reorganizing primary care delivery models to manage care
6. Selecting primary care physician-hospital alignment models and approaches
