The industry’s largest awards program for case managers and care coordinators is open to leading individuals and organizations.

The program aims to provide visibility of the practice of case management to the public, to lawmakers and other members of the healthcare team so they recognize the value they bring as critical members of the team.

The publisher of Case In Point, the leading resource for healthcare professionals working in the broad scope of care coordination, is accepting entries for the 4th annual Case In Point Platinum Awards, a national awards competition and ceremony that recognizes excellence in case management, care management and care coordination across the healthcare spectrum.

The Case In Point Platinum Awards recognize the most successful and innovative case management programs and individuals working to improve healthcare across the care continuum. Today, the practice of case/care management is viewed across the healthcare industry as a key process to improve efficiency and effectiveness in order to improve quality and contain escalating costs.

This year’s Platinum Awards program includes a total of 38 categories that highlight the full spectrum of case management programs as well as individuals working on the frontlines of managing care – from best acute care case manager to best embedded case management program with dozens in between. The Platinum Awards Program seeks to honor individuals and programs that best educate and empower patients, improve adherence to care, and engage patients in meeting their individual healthcare goals.

This Platinum Awards Program provides the opportunity for organizations and professionals to celebrate their successes in a way that elevates the practice to all stakeholders.

“Now in its fourth year, the Case In Point Platinum Awards is the only awards competition in the healthcare industry that is all-inclusive and provides a platform to publicly showcase the important being done to improve the delivery of care across the healthcare landscape,” said Anne Llewellyn, editor in chief of Case In Point. “The program aims to provide visibility of the practice of case management to the public, to lawmakers and other members of the healthcare team so they recognize the value they bring as critical members of the team.”
