Tunstall Americas has reached an agreement with Faith Tele-Health (FTH) dba Concierge Care Transition to embark on a pilot program together of delivering a new model of transitional care service that combines traditional personal emergency response (PERS), telephone concierge services and outbound transitional care calls to a pre-qualified group of post-discharged hospital patients. The aim of the pilot is to improve patient outcomes, reduce hospital emergency room visits and hospital readmissions by providing as much non-clinical support as necessary to enable them to follow discharge orders.

We know that a select population has difficulty transitioning from hospital to home, whether it be something as simple as obtaining their prescription to something much more crucial such as missed doctor appointments. Additionally, the PERS unit provides the patient with peace-of-mind knowing that we are here for them 24/7,” said Brad Waugh, President and CEO of Tunstall Americas.
The FTH Concierge Care Transition Coach will be the patient’s initial post-discharge point of contact. They will meet with the patient, provide real-time concierge services, install the Tunstall PERS unit and educate the patient on how to contact Tunstall’s concierge and emergency response agents.
Statistics show that early intervention improves patient outcomes. This transitional care service complements FTH Concierge Care Transition’s current services by providing real-time information to the patient’s clinical care team therefore enabling faster response time and improving the likelihood of a quicker recovery.
Beginning in mid-November, Tunstall Americas and FTH Concierge Care Transition will be piloting this program in a major New York hospital. The pilot is expected to last 5 months and will include participation of 100 discharged patients.
