A pioneer and leading provider of Population Health and Business Intelligence solutions, i2i Systems announced today that i2iTracks 7.3 has achieved NCQA PCMH Prevalidation status. NCQA recognizes i2iTracks as a solution with functionality that completely meets “specific” factor requirements in the PCMH 2011 Recognition survey. i2iTracks supports several PCMH standards including Managing Populations, Tracking Referrals and Measuring and Improving Performance. i2iTracks 7.3 received PCMH Prevalidation status after completing an extensive review process with NCQA.

i2i Systems provides practices with an all-in-one package of approved Population Health Management technology and documented processes to achieve parts of PCMH recognition. Providers, for example, can use i2iTracks to demonstrate using data for managing care of a specific population around chronic illness. In the case of diabetes, the provider can set up a filter in i2iTracks and search all patients with an HbA1c at risk. The search produces a contact list for outreach, where providers can correspond with their diabetic patients, encouraging them to come in for a foot check, eye exam or other service.
