Ingenious Med, the nation’s largest multispecialty charge capture and physician performance solution, has released a white paper, “Closing the Safety Gap in Patient Handoffs: Leveraging Technology to Build the Safety Net.”
According to the report, written by Dr. Glenn Focht, medical director at Boston Children’s Hospital, “The days for rushed, scribbled handoffs or a quick phone call must end if patients are to be cared for safely. At the same time that this clinical need is growing, the time demands for hospitalists, house officers and hospital-based providers are increasing.”
The paper examines the breakdowns in communication that take place during patient handoffs, often causing harm to the patient. According to a 2008 study cited in the paper, 58.3 percent of house officers reported at least one patient experienced minor harm during their most recent rotation due to problems with handoffs.
“Hospitalized patients will experience less harm only when physicians are successful in managing the growing number and complexity of handoffs,” said Focht. “As an initial step, organizations must embrace and endorse the best clinical practices for effective handoffs.”
The paper discusses four best practices that ensure safe, effective and sustainable handoffs:
