PCH Mutual Ins. Co., A RRG is pleased to announce the extension of a new contest for assisted living facilities, adult care homes, and their employees. PCH Mutual, Ins. CO, A RRG is looking for assisted living facilities and adult care homes and their employees that have implemented successful fall prevention plans in their operations.
The contest start date is March 1, 2013 and will be extended to end on April 15, 2013. Submissions can be e-mailed to contest(at)pcalic(dot)com or mailed to “Fall Prevention Contest, C/O PCALIC, LLC, PO Box 933, Hanover, PA 17331” and should include the following information:
1. Your name, address, email address, and telephone number
2. Name, city and state of the Facility/Home where the fall prevention plan was successfully implemented
3. Your fall prevention policies, procedures, and pictures if needed
4. Description of why your fall prevention plan is successful (50 word maximum)
Submissions will be voted on between April 1st and April 15th of 2013. We are inviting all licensed assisted living facilities and adult care homes, excluding residents of New York, Florida, and Rhode Island to participate to win one of the following prizes:
GRAND PRIZE: $1,000 Cash Prize
RUNNER-UP: $500 Cash Prize
