Parrish Medical Center has launched a free mobile app with information and tools needed to manage day-to-day health. Among other benefits, the app provides interactive health trackers that allow a person to monitor weight, glucose, blood pressure, medication adherence, and other metrics that are essential to health and wellness.

The health app is available for download at and includes health libraries from the Mayo Clinic, which covers more than 2,000 diseases and conditions. Persons with chronic conditions, such as diabetes, hypertension, and heart disease, can use the Mayo Clinic content, along with the health-tracking tools, to assist them in managing their conditions day-to-day.

HealthBridge is PMC’s initiative to provide the right information, to the right patient at the right time to transform the health care experience for those it serves. This mobile app is an additional benefit for PMC’s large community of HealthBridge members. Membership into the HealthBridge program is free and allows community members to personalize health information according to their specific needs.

Parrish Medical Center partnered with Axial Exchange to develop the app. Axial’s software solutions can be found in more than 30 hospitals across the country, and focuses on improving patient engagement and clinical outcomes. Axial won first prize in the US Department of Health and Human Services’ Partnership for Patients initiative on the Ensuring Safe Transitions From Hospital to Home Challenge run by the Office of the National Coordinator (ONC).
