Telligen announced today the results of an independent evaluation of the SoonerCare Health Management Program which it administers for the Oklahoma Health Care Authority, the state’s Medicaid agency. The evaluation results show the program has saved the state of Oklahoma $139.2 million during the past four years. The evaluation also shows that Telligen was able to improve the quality of care given by participating providers and has achieved high levels of patient and provider satisfaction with the program.
The evaluation of the health management program by the Pacific Health Policy Group of California focused on the following components; care management and practice facilitation. The report found that the program has generated more than $6 in medical savings for every $1 invested in program administrative costs, an increase from a $4:1 return in 2011. The Care Management component of the program decreased monthly medical expenses for patients enrolled in the program ranging from 18-28% below predicted levels, saving Oklahoma taxpayers $93.1 million. The practice facilitation portion of the program helped participating providers increase their compliance with evidence-based clinical guidelines, resulting in improved quality of care and saving the state an additional $46.1 million.
The statewide SoonerCare Health Management Program was established in 2008 and incorporates Telligen’s Population Health Management solution. Under this model, Telligen health coaches work one-on-one with more than 5,000 chronically ill patients and their physicians to develop individualized care plans, build self management skills, improve health literacy, assist with medication management and help patients become more knowledgeable and engaged consumers of healthcare services. In addition, Telligen embeds practice facilitation experts in physician offices to examine care delivery and business practices and to recommend evidence-based changes that result in better clinical outcomes for patients.

In addition to improved quality and cost savings, the program evaluation also revealed other positive trends:
- 91 percent of providers who completed practice facilitation viewed the program favorably and would recommend the program to other providers.
- 75 percent of providers credited practice facilitation with having a positive impact on enrolled patients.
- High-risk patients spent 65 percent fewer days in the hospital that they typically would have.
- Participating physicians were able to reduce claims costs by 11.5 percent.
- Monthly medical expenditures per member declined in each of the three years following coaching, ranging from 10 percent to 30 percent per year.
- 90 percent of members were satisfied with their health coach.
Due in part to the success of the program, the Oklahoma Health Care Authority has recently awarded Telligen a second contract to administer the SoonerCare program through Fiscal Year 2018.
