Virginia based Centra Health has selected HealthMEDX Vision, along with the Exchange Platform, as their complete health information technology solution. Driving this decision is Centra’s commitment to provide technology that will seamlessly manage patient care across all post-acute care settings, as well as enable the exchange of patient information across their entire acute-care continuum, including hospitals, physician offices, pharmacies and labs. Centra’s skilled nursing facility (SNF) provides continued care for patients discharged from the hospital that require transitional care. Through Centra’s innovative cardiac program, a specially-trained cardiac nurse manages care for cardiac patients in the SNF using specialized care pathways developed by Centra’s cardiology group. Since implementation of the cardiac program in the post-acute setting, Centra has reduced patient length of stay from 4.52 days in 2006 to 3.85 days in 2012.
Based on this success, clinical leaders at Centra are developing similar programs for orthopedics and piloting Emergency Room Diversion Protocols. Centra leaders believe it imperative that their post-acute EMR technology be able to manage care across all post-acute care settings, as well as interoperate with the full acute care continuum.
