As our elderly population rises to record levels, more people are becoming caregivers for aging parents and loved ones. Veteran caregivers, Barbara Stephens and daughter, Tanya Choate, have created a new online resource that offers family and in home caregivers a wide variety of medical and household forms to take the chaos and confusion out of caregiving. "Caregivers are like any other workers. They need to have the right tools to do a good job. Working without the proper record-keeping is just frustrating," says Tanya, "The Caregiver Forms website is a toolbox for caregivers."
Barbara owned and operated a residential care facility for 12 elderly residents. To keep organized, she began designing her own caregiver forms. Residents, family members, doctors, pharmacists, and home health providers have found Barbara’s forms a quick and easy way to keep all the information organized.
Tanya began using her mother’s forms while caring for her diabetic grandmother. One day she called 911 during a crisis with her grandmother. When the paramedics arrived, Tanya handed them her grandmother’s Medication List. The paramedics were very enthusiastic about how much easier that one form made their job.
Tanya said, “They told me, if everyone had a medication form, it would save them a lot of time and possibly help avoid dangerous drug interactions. I knew then we had to do something to get the forms out to the public through the internet." says Tanya.
Caregiver Forms that are, easily downloaded and printable include; the Medication Form, daily care checklists, menu planners, expense forms, time sheets, Caregiver or family work schedules. Tips for record keeping, organization, negotiation, and support are also available. Prices range from $2 for individual forms to the entire packet of forms for $22.
