The process for renewing a medical marijuana card begins with, according to Valley Evaluation Centers in compliance with Arizona’s Medical Marijuana laws and the Medical Marijuana program guidelines, a visit to the doctors office. Like the initial steps to gaining a medical marijuana card, one must receive a recommendation from a physician licensed to practice medicine in the state of Arizona.
A medical marijuana card is valid for only a year and must be renewed annually. Valley Evaluation Centers advises patients to be aware of this, and the expiration date of their card (which can be found on the front of the card). The state of Arizona allows 90 days prior to the expiration date for card renewal.
Qualified patients must fill out an application and submit it online to the Medical Marijuana Department. This will include a completed Medical Marijuana Physician Certification form, a Medical Marijuana Patient Attestation form, a new picture, and a copy of your Arizona I.D.
