The organization which administers the Medicare program, The Centers for Medicare & Medicaid Services (CMS) has instituted a policy to reduce all Medicare reimbursements to a particular hospital if they are deemed to have a higher readmission rate within 30-days of discharge than regional averages. This reduction could be as much as 2% of a hospital's total Medicare reimbursement, which for some hospitals is close to 50% of their total revenues.
Many of the hospitals in Northeast Florida are experiencing a reduction to some degree, and almost all are taking proactive steps to reduce readmissions. The Health Planning Council of Northeast Florida is now putting its large data sets, which are void of personally identifiable information, to use, by building a predictive model which can be used to “score” the probability that a patient will be readmitted within 30-days.

The Health Planning Council (HPC) has entered into an agreement with Jacksonville, FL–based NLP Logix, to build and host the predictive model. NLP Logix will apply machine learning to the massive data sets the HPC has collected since being chartered by the State of Florida 30 years ago. “We will be able to learn from not only the region, but the entire state of Florida, what attributes effect the probability of a readmission for specific patients,” said Matt Berseth, Chief Scientist at NLP Logix. “The power and richness of the data set is incredible and can be used to really help hospitals identify risk by learning from the past experience of all the hospitals in the state.”
The program is expected to be in the test phases by the end of August, 2013 and in full production and available to the entire region by October 1, 2013. “We are excited to be able to provide predictive reporting back to our members,” said Emerick “the 30-day readmit is just the first, there are many more models in the works that we intend to roll out over the next couple of years.”
