HomeTown Health, LLC in partnership with the Florida state office of rural health are working with rural hospitals in a quality improvement initiative designed to support and enhance the quality of care of these healthcare centers. The facilities serve mostly rural communities and face a unique set of operational challenges. Originally created to serve the needs of this specific group, this project, along with the additional resources and services made available through the partnership with Hometown Health have the potential to expand further into these facilities with a broad spectrum of application possibilities.
Through this initiative, the facilities are provided with tools to help measure performance, identify areas that can improve and connect with resources to complete that process. Sharing Best Practices and identifying common issues that can become additional opportunities for collaboration are in important piece of this initiative. With the successful identification of new areas of focus as this partnership continues with additional grants, the collaborative effort is already creating positive results. Additional resources available to the participating hospitals include exclusive courses, customized databases for performance measures, access to webinars and educational opportunities specifically designed to address quality improvement issues in small hospitals, vendor discounts, and on-site hospital visits and staff support from HomeTown Health team members.
The SHIP grant, or Small Hospital Improvement Program, is designed to help small (49 beds or less) facilities in mostly rural areas with needs unique to their issues. The current focus is preparing hospitals for changes associated with implementation of the Affordable Healthcare Act.
