Wellcentive, an Atlanta-based provider of innovative Population Health Management and Data Analytics solutions, today announced that it has been selected by ProMedica’s Michigan-based Lenawee Physician Hospital Organization (LPHO) to support its various clinical quality and performance-based initiatives, including Pay-for-Performance (P4P) and Patient-Centered Medical Home (PCMH) programs, as well as to help it prepare to become a clinically integrated network.

ProMedica’s LPHO signed a three-year contract after an extensive review of more than a dozen competing firms. The organization, representing 110 physicians, will utilize Wellcentive’s Outcomes Manager and Data Analytics solutions to manage its diverse and extensive inventory of incentive initiatives. This grouping of initiatives includes Blue Cross Blue Shield of Michigan’s Organized Systems of Care (OSC) and Physician Group Incentive Program (PGIP), as well as the Michigan Primary Care Transformation Project (MiPCT) and Partners in Performance (PIP) program.

The Wellcentive implementation for ProMedica’s LPHO will include 16 different interfaces. Some of these interfaces will help aggregate and normalize data from various data sources such as electronic health records (EHRs) and practice management systems (PMSs), and other interfaces will send data directly to payers to help ease the reporting burden for different P4P programs.
