The doors of Bend Transitional Care (located at 900 NE 27th Avenue Bend, OR 97701) will have been open to the public just a year this August and already the post-hospital care facility is receiving national attention for its dedication to quality care.

The transitional care facility is designed to rehabilitate patients after being discharged from the hospital. Health inspections, nurse staffing, and quality measures are the three categories the Centers for Medicare & Medicaid (CMS) look at when the publication determined Bend Transitional Care deserved an overall rating of five stars. U.S. News and World Report’s Best Nursing Homes 2014 examined the ratings of nearly 16,000 facilities across the country and honored Bend Transitional Care as one of the few top care centers in the nation.
