Health Dialog, a leading provider of population health management solutions, announced today a multi-year agreement with CDPHP, a New York-based health plan that recently received the National Committee for Quality Assurance’s (NCQA) highest ratings for quality care. Under the terms of the agreement, Health Dialog will provide its award-winning solutions for disease management, health risk assessment, and HEDIS® support to CDPHP’s more than 450,000 members. The relationship extends an existing collaboration of nearly a decade, which was designed to help CDPHP members make good healthcare decisions based on the best medical evidence available.
Under the agreement, Health Dialog will continue to work with CDPHP to provide patients with one of the most complete approaches to disease management. CDPHP members will have access to Health Dialog’s complete library of shared decision making aids, one-on-one health coaching and personalized wellness initiatives. Combined, these programs give CDPHP members comprehensive support along the path to better health. Data from a Health Dialog member satisfaction study show that improved learning combined with interactive and collaborative discussion with both a health coach and the treating physician can help patients make better healthcare decisions suited to personal values and lifestyle.
