The High Cost Patient Work Group, part of the Health Care Transformation Task Force (HCTTF), a powerful new private-sector alliance dedicated to accelerating the transformation of the U.S. health care system to value-based business and clinical models aligned with improving outcomes and lowering costs, is releasing a paper today entitled “Proactively Identifying the High-Cost Population.”
This paper serves as a guide to help providers and payers define different types of high-cost patients, understand their clinical characteristics and learn more about methods that may be used to identify and stratify high-cost patients for care management.
The HCTTF is an industry consortium that brings together patients, payers, providers and purchasers to align private and public sector efforts to clear the way for a sweeping transformation of the U.S. health care system. The HCTTF is committed to rapid, measurable change, both for ourselves and our country. The HCTTF aspires to have 75% of participating organizations operating under value-based payment arrangements by 2020.
