“Our relationship with UnitedHealthcare will improve access to care for more diverse patients in our community, reduce disparities in health outcomes and improve people’s health and well-being," said Hugh Lytle, Equality Health Founder and CEO.
Phoenix, Arizona (PRWEB) October 04, 2017
Equality Health and UnitedHealthcare launched a new program to improve the quality of health care for people enrolled in UnitedHealthcare’s Medicaid plans in the Phoenix area by promoting a team-based, holistic approach to patient care.
Through this program, Equality Health’s integrated network of physical, behavioral and community-based care providers will deliver quality, culturally effective and coordinated health care to their patients. The collaboration improves the ability to identify social and cultural factors that put patients at high risk and help them reduce avoidable emergency room visits and readmissions to the hospital, manage chronic health conditions and take their needed medications.
Equality Health has developed its innovative approach to care management using technology, services and network designs to organize a better health care delivery system for diverse communities that have struggled with integrating into the traditional one-size-fits-all system.
“At Equality Health, we are building new cultural care pathways to create better outcomes, increase patient adherence and lower the cost of care for underserved communities,” said Hugh Lytle, Equality Health founder and CEO. “Our relationship with UnitedHealthcare will improve access to care for more diverse patients in our community, reduce disparities in health outcomes and improve people’s health and well-being.”
“UnitedHealthcare and Equality Health are committed to ensuring people who live in underserved communities have access to the quality, culturally competent care they need and deserve,” said Joseph Gaudio, CEO, UnitedHealthcare Community Plan of Arizona. “By collaborating with Equality Health, we can create a more connected health care experience, help improve patient care and enhance people’s ability to live healthier lives.”
More than 15 million people enrolled in UnitedHealthcare plans across the country have access to accountable care programs, delivered in part through more than 800 accountable care arrangements nationwide as the organization engages in deeper, more collaborative relationships with physicians and hospitals.
