“Revisional Obesity Surgery Endolumenal (ROSE) is very new,” said Dr. Michael Ujiki, who has extensive training in the procedure. “There are approximately 9 centers in the United States performing the procedure and we are the first in the region. Given the number of patients who have undergone gastric bypass in our region over the last 20 years, we predict that the ROSE procedure will become a popular and effective tool in the fight against obesity.
Every year more than 100,000 U.S. patients undergo gastric bypass surgery for the treatment of obesity. Experience now shows approximately 20 percent of these patients will regain weight within a few years after the surgery, due to the stretching of the stomach, and will be at renewed risk for diseases.
Ujiki adds, “The ideal candidate is one that is at least two years out from gastric bypass, who originally lost weight, and is now regaining that weight. A lot of patients who regain weight, unfortunately also suffer recurrences of those diseases associated with obesity such as diabetes, hypertension and sleep apnea. The ROSE procedure should result in weight loss and resolution of these obesity-related conditions.”
The ROSE procedure uses instruments inserted through the mouth to reduce the size of a patient’s stomach pouch and the opening to the small intestine to help patients achieve weight loss again. To perform the scarless procedure, a small, flexible endoscope and surgical tools are inserted through the mouth and into the stomach pouch. The tools are used to grasp, fold and stitch tissue to reduce the diameter of the stomach opening and the volume of the stomach pouch. No cuts are made into the patient’s skin during the procedure.
“Surgeons do not have to work around or through scar tissue formed from the previous surgery,” Ujiki said. “Scar tissue can be very difficult to a surgeon and ultimately the patient, because it can lead to increased incidence of injury to nearby structures, decrease visualization, and make handling of the stomach difficult. Revising the gastric pouch through the mouth from the inside avoids these problems.”
Most patients will return home the same day of the surgery and return to work the next day. More than 15 million people in the United States suffer from severe obesity. Surgical treatment of obesity has increased significantly in recent years and it is estimated that over 125,000 patients today are candidates waiting for a revision procedure.
