The Heritage Woods of Aledo affordable assisted living community is offering a special Healthy Living program for older adults with chronic medical conditions, starting on Thursday, Nov. 1.
The “Take Charge of Your Life: Live Well, Be Well” program was developed by the Patient Education Research Center at Stanford University as a research project between Stanford and the Northern California Kaiser Permanente Medical Care Program.
Two trained leaders will be conducting the six-week program, with participants meeting on Thursday afternoons at Heritage Woods of Aledo, 405 SE 13th Ave. in Aledo, Illinois.
Space is limited so call Danise Lenz at 309-582-1132 to reserve a spot or for more information.
“During the sessions, participants will learn about building a more effective relationship with their doctor; dealing with pain, isolation and frustration; and communicating with family and friends,” says Lenz, who is Director of Marketing for Heritage Woods of Aledo. “Tips on eating, exercise and medication management also are covered, and a companion book entitled “Living a Healthy Life with Chronic Conditions” is used.
The program is designed for those with chronic conditions such as arthritis, heart disease, asthma, lung disease, diabetes, emphysema and Parkinson’s disease, says Lenz.
