With health care constantly evolving, it’s vital health care organizations seize opportunities to innovate and to adopt processes that support efficient, patient-centered care.

Today, HealthyCircles, an integrated suite of online Professional and Consumer portals, officially announces open enrollment for their Wednesday, September 21, 2011, webinar on Providing Cost-Effective Care Navigator Programs and Infrastructure at 11:00am PDT. Please note, space is limited for this exclusive event, so please e-mail webinars(at)healthycircles(dot)com to reserve your spot today.

For a patient or a family, navigating the health care system is often difficult and leads to frustration and gaps in care. For example, a December 2010, BMJ Quality & Safety article “The Quality of Cancer Patient Experience: Perspectives of Patients, Family Members, Providers and Experts”, found that patients who had regular visits with their providers had improved outcomes. Thus, it is vital to ensure patient navigation is a top priority.

In addition, with new payment models, it is imperative organizations focus on adopting the innovative models that support patient navigation. Implementing new models of connected health now will enable Care Teams to leverage the principles of connected health while reducing hospital readmissions, increasing care coordination, improving outcomes and driving patient satisfaction.

“When the care navigation process runs smoothly, professionals benefit from better communication and access to information that can impact redundant testing. More importantly, the patient and family reap the benefits of superior care, active engagement and increased knowledge,” stated Susan Nedza, MD, MBA, FACEP, Vice President of Strategic Clinical Solutions at HealthyCircles. “With health care constantly evolving, it’s vital health care organizations seize opportunities to innovate and to adopt processes that support efficient, patient-centered care.”

“HealthyCircles is proud to host this webinar on care navigation as it’s a topic that affects so many. From the moment a patient learns they have cancer, for example, they may feel overwhelmed, confused and frustrated by the lack of information and coordinated care,” stated James Mault, MD, FACS, Chairman and Chief Executive Officer of HealthyCircles. “This frustration often continues through the entire care continuum, leaving a dissatisfied patient and increased costs due to duplicate procedures and medical errors, and it’s the patient that suffers the most from these inefficiencies. Our goal is to provide a care navigator system that alleviates these issues.”

HealthyCircles, with guest speaker Dr. Susan Nedza, will host Providing Cost-Effective Care Navigator Programs and Infrastructure on Wednesday, September 21, 2011 at 11:00am PDT. Attendees from organizations seeking the most innovative and practical solutions to improve patient care navigation are encouraged to attend. Space is limited, so please e-mail webinars(at)healthycircles(dot)com to confirm your registration today.
