PSCI Solutions reported a record year since the launch of its products in the healthcare marketplace. PSCI‘s Next Generation Population Predictive Model is a four-tiered model to identify patients with higher risk of hospitalizations, and integrates:

  •     State-of-Health (SOH) Models: These are the core of prediction models for each disease condition harvesting the latest vitals and test results from EMR data. These models do not change with population (Medicare, Medicaid, commercial).
  •     Utilization Predictive Models: Analyze the claims data to capture the historical healthcare resource consumption patterns.
  •     Compliance Models: Capture the level of compliance of patients to according to accepted guidelines and best practices – i.e., doctor visits, pharmacy utilization compliance, etc.
  •     Access-to-Care Models: Capture the socio-economic determinants that impact the risk of hospitalization.

PSCI’s patent-pending solution to prediction helps in identifying the causal factors for calculating the patient’s high risk score, which are used to develop a tailor-made care management program. PSCI has developed models for chronic conditions, pediatric conditions and continues to build models in specialty conditions like urology, cardiology, etc.
